HORSE SHOE KIDNEY


                                                HORSE- SHOE KIDNEY

  • During development, 2 mesonephric buds appear on the side of the future vertebral column and grows into metanephros.
  • Mesonephric buds form ureter and metanephros kidney.
  • If fusion occurs at lower pole, it results in Horse- shoe kidney.
  • Upper pole fusion results in reverse horse shoe kidney which is rare.
  • Inferior mesentric artery crosses the isthmus at the level of L3- L4.
  • MC site is infront of 4th lumbar vertebrae.
 

CLINICAL FEATURES-

  • Asymptomatic for many years
  • More common in males
  • Present as fixed, nonmobile firm mass in the midline of 4th lumbar vertebrae.
  • Recurrent UTI
  • Ureteral Obstruction is common
  • More prone for infection, stone formation, hydronephrosis, TB
  • Associated anomalies are spina bifida, congenital hemivertebrae, Turner’s syndrome, cleft lip and palate.
  • Rovsing’s sign- abdominal pain, nausea, vomiting.

INVESTIGATIONS-

  • USG- locate kidney
  • CT scan or isotope renogram- confirms horse shoe kidney
  • IVU- medialisation of lower calyces curving of ureter like Flowervase appearance.
 

TREATMENT-

  • Complication are treatment
  • Removal of stone or repair and reconstruction of hydronephrosis are done.

Exam Important

  • If fusion occurs at lower pole, it results in Horse- shoe kidney.
  • Upper pole fusion results in reverse horse shoe kidney which is rare.
