SOLITARY THYROID NODULE

  • STN is the presence of single palpable nodule in the thyroid gland while remaining gland is not palpable.

ETIOLOGY-

  • Part of MNG (dominant nodule)
  • Puberty goiter
  • Colloid goiter (MC)
  • Toxic adenoma
  • Thyroid cyst and carcinoma
  • Thyroiditis

CLINICAL FEATURES-

  • Seen in females more commonly
  • STN has higher malignancy rate than MNG.
  • Asymptomatic swelling
  • Characteristic feature is it moves on swallowing.
  • Most common surgical disease of thyroid gland.
  • Solitary solid swelling in a male has higher risk of malignancy.
  • Dysphagia
  • Perisistent cough or stridor
  • Hoarseness due to pressure on recurrent laryngeal nerve or superior vena cava obstruction.

INVESTIGATIONS-

  1. Thyroid function test is the first investigation including TSH, T3, T4
  • TSH test is ultrasensitive and can detect subclinical hypothyroidism and hyperthyroidism.
  1. FNAC is the definitive investigation
  2. USG- swelling is solid or cystic, detects nodules of MNG, lymph nodes
  3. Thyroid scan-
  • Uses  131I or Technetium- pertechnetate 99m
  • On scanning swelling can be-

a) Hot (overactive)

b) Warm (active)

c) Cold (underactive)

  • Detects difference between toxic adenoma from toxic MNG.

INDICATIONS FOR SURGERY-

  • Malignant cytology
  • Large size, substernal excision
  • Cosmesis, male sex, recurrence of cyst
