Sepsis with ARDS
Nitrofurantoin can cause acute hypersensitivity pneumonitis which is characterized by diffuse inflammation of lung parenchyma and airways and typically presents with fever, chills, cough, and wheezing.
It is treated by discontinuing the causative agent and with oral steroids.
The most common form occurs in farmers and cattle workers. It is due to actinomycetes.
Sepsis with ARDS would not produce the rash or eosinophilia.
