Hypoplastic left heart syndrome and critical aortic stenosis have a dependent systemic circulation. So, as soon as the duct starts to close, the cardiac output decreases, and symptoms progress rapidly causing cardiogenic shock and congestive cardiac failure. In these cases, PGE2 analogs which keep the duct patent can be used till the baby is posted for surgery.
