AIDS Control Programmes


WHO case definition for AIDS:
  • In an adult or adolescent (>12 years)
Major Signs:
  • Prolonged fever more than 1 month
  • Chronic diarrhoea > 1 month
  • Weight loss > 10% of body weight

Minor signs

  • Persistent cough for more than one month
  • Generalized pruritic dermatitis
  • History of herpes zoster
  • Chronic progressive or disseminated herpes simplex infection
  • Generalized lymphadenopathy
  • Oropharyngeal Candidiasis.

Expanded WHO case definition for AIDS surveillance

  • For surveillance ,adult or adolescent (>12 years of age) is considered to have AIDS if a test for
  1. HIV antibody gives a positive result and one or more of the following conditions are present :
  2. >10% body weight loss or cachexia, with diarrhoea or fever or both, for at least 1 month, not known to be due to a condition unrelated to HIV infection.

National AIDS control Programme

  • Providing 3 million people treatment by end of 2005 was “3 by 5” initiative .
  • In 1992 NACP I was implemented with objective of slowing down spread of HIV.
  • 1999, NACP II implemented
  • Objectives of NACP II:
  1.  To reduce the spread of HIV infection in India
  2.  To increase India’s capacity to respond to HIV/AIDS on a long-term basis
  • In April 2002, NACP III launched
  •  The primary goal of NACP-III is to halt and reverse the epidemic .
  • February 12, 2014 launched NACP VI, under the banner of NACO.

Highlights of NACP IV:

  • Reversal of AIDS epidemic
  • Targeting High Risk Groups (HRG) – truckers, migrants, pregnant women, etc
  • Promotion of condoms (free distribution) in particular female condoms .
  • Social protection.

Blood safety programme

  • Guidelines for blood bank, blood donors and dialysis unit  formulated.
  •  Strategy is to ensure safe collection, processing, storage and distribution of blood and blood products.
  • Testing of every unit of blood is mandatory for ->HIV, HBV, HCV, malaria, syphilis
  • Objective is to ensure reduction in the transfusion associated H1V transmission to less than 0.5%.

STD control programme

  • Diagnosis and treatment of STD major strategies to control spread of HIV infection.
  • For treatment of STDs, blue colored pack is used for treatment of Genital ulcer

HIV surveillance –

  • HIV sentinel surveillance
  • HIV sero-surveillance
  • AIDS case surveillance
  • STD surveillance
  • Behavioral surveillance

School AIDS educational programme

  • Training module called Learning for life has been prepared to bring an uniformity in approach

 Anti-retroviral treatment (ART)

  •  91 ART centres
  • Combination of three potent drugs,  given to the persons with advanced stage of AIDS.
  • ACO has branded services as “Suraksha clinic” .

 Prevention of parent to child transmission 

  • HIV positive women given single dose of prophylactic Nevirapine at time of labour 
  • Newborn infant is also given a single dose of nevirapine within 72 hours of birth. 

 National paediatric AIDS initiatives 

  • Provision of ART to children infected by H1V. 
  • Drugs are provided at all ART centres.
  • Free CDH monitoring, free DNA-PCR testing for children upto 18 months
  • Diagnosis & treatment of opportunistic infections
  • Micronutrient supplementation.

MILLENNIUM DEVELOPMENTAL GOAL:

Goal 6: Combat HIV/AIDS, malaria and other diseases:
  • IV prevalence among young people aged 15 to 24 years
  • Condom use rate of the contraceptive prevalence rate
  • Number of children orphaned by HIV/AIDS
  • Prevalence and death rates associated with malaria
  • Proportion of population in malaria-risk areas using effective malaria prevention and treatment measures
  • Prevalence and death rates associated with tuberculosis
  • Proportion of tuberculosis cases detected and cured under (DOTS)
The Red Ribbon Express
  • Seeks to spread awareness about HIV/AIDS
  • Promote safe sex 
  • Reduce discrimination against AIDS victims in India, especially those in rural regions. 
  • NACO launched the Red Ribbon Express to commemorate World AIDS Day(dec 1).
  • 8 coach exhibition train travel to 22 states covering 152 halt station during its year long journey. 

Integrated counselling and Testing centres (ICICs)

Established at :

  1. Medical colleges 
  2. Sub-district level hospitals
  3. District hospitals 
  4. Community health centres.
  • Iis proposed to exend services to 24 hours PHCs.
