Grading of Laryngeal View

  • The Cormack-Lehane system classifies views obtained by direct laryngoscopy based on the structures seen.
  • It was initially described in 1984 as a way of simulating potential scenarios that trainee anesthetists might face.

The new classification subdivides grade 2 and 3.

  • Grade 2a: Posterior Cords Visible
  • Grade 2b: Only Arytenoids visible
  • Grade 3a: Epiglottis visible and liftable
  • Grade 3b: Epiglottis adherent to the pharynx.
    • The view is easy(Grade 1 and 2a): suitable for intubation under direct vision.
    • The view is restricted (Grade 2b and 3a): likely to benefit from indirect methods (e.g. gum elastic bougie).
    • A difficult view (Grade 3a and 4): likely to need specialist methods for intubation which may need to be performed blindly.
