HAEMOBILIA

  • Bleeding from liver into biliary tree.
  • Communication between blood vessel and bile duct.

 

ETIOLOGY—

  • Accidental trauma (iatrogenic trauma)
  • Malignant liver disease
  • Portal hypertension
  • Gall stones
  • Vascular disease of hepatic artery

 

CLINICAL FEATURES-

  • Sandblow/ Quinck’s traid- jaundice, pain and meleana (most common)
  • Pain abdomen- colicky
  • Haemorrhage

 

INVESTIGATIONS-

  1. LFT
  2. USG abdomen
  3. The diagnosis is confirmed by a side-viewing duodenoscope to identify bleeding from the ampulla 
  4. Selective arteriography
  5. Upper GI endoscopy (1st IOC of meleana)

 

TREATMENT-

  • Selective arterial embolization (angiographic)
  • Open surgical ligation of bleeding vessel
