This is a CT Scan image of Primary CNS lymphoma
Ans. D. All- ‘a’ i.e., Reticulin staining is done; ‘b’ i.e. Essentially B cell type; ‘c’ i.e., Have an association with EBV
Primary CNS lymphoma (PCNSL) are uncommon tumours, accounting for only 1% of malignant CNS tumours.
- PCNSL is a densely cellular, aggressive non-Hodgkin B-cell lymphoma.
Imaging:
- On imaging, PCNSL characteristically is identified as a CT hyperdense enhancing supratentorial mass.Haemorrhage is distinctly uncommon.Surrounding Vasogenic edema may be seen. There are often multiple lesions in patients with HIV/AIDS.
- On MRI T1 hypointense, T2 iso- to hyperintense, vivid homogeneous gadolinium-enhancing lesion/s with restricted diffusion, subependymal extension, and crossing of the corpus callosum is seen.
- Enhancement on both CT and MRI is pronounced and usually homogeneous.
Histology-Pathology:
- Typical Primary CNS Lymphoma demonstrate a striking angiocentric pattern, particularly at the edge of the mass.
- Blood vessels are seen, and these are surrounded by cuffs of malignant lymphocytes.
- A special stain for Reticulin often shows deposition of this material in concentric layers around the blood vessels
Predisposing factors :
- HIV/AIDS: approximately 2-6% of patients with HIV will develop PCNSL
- Prior EBV infection
- Post transplantation
- IgA deficiency
- Wiskott-Aldrich syndrome.
