ACUTE PSYCHOTIC DISORDERS:
- Disorders with symptoms (delusions, hallucinations & disorganization symptoms) similar to schizophrenia.
- Disorders frequently preceded by stressor (stressful life event) – Precipitated by fever.
- Acute onset & often resolve completely.
- Classification:
- Classified separately in DSM-5 & ICD-10.
- According to DSM-5 –
- Symptoms present for
- (Note – Symptoms for 1-6 mons – “schizophreniform disorder”).
- According to ICD-10 –
- Symptoms present for mon – Classified as “Acute & transient psychotic disorder”.
- Treatment: Antipsychotics & benzodiazepines.
SCHIZOAFFECTIVE DISORDER:
- Features of both schizophrenia & mood disorders concurrently.
- Types:
- Depending on manic episode or depressive episode along with schizophrenia symptoms.
- Schizoaffective disorder (Bipolar/manic type) – With manic symptoms.
- Schizoaffective disorder (Depressive type) – With depressive symptoms.
- Treatment –
- Combination of mood stabilizers, antipsychotics & antidepressants – Depending on presentation.
- Combination of antipsychotics & mood stabilizer – For schizoaffective (manic type episodes).
- Combination of antipsychotics & antidepressants – For schizoaffective (depressive type episodes).
DELUSIONAL DISORDER:
- Characterized by development of single delusion or set of related delusions.
- Usually persistent & life-long.
- Psychotic symptoms (hallucinations, disorganization, negative symptoms) – Absent.
Types:
- Persecutory type: Delusion of persecution.
- Jealous types: Delusion of infidelity.
- Erotomanic type: Delusion of love.
- Somatic type:
- Subtypes:
- Delusional parasitosis – Delusion of infested by parasites.
- Delusion of dysmorphophobia – Delusion of misshaped body parts.
- Delusion of halitosis – Delusion of body has a foul odor.
- Grandiose type: Delusion of grandiosity.
- Unspecified type: Above mentioned categories unapplicable.
- Eg: Delusion of misidentification.
SHARED PSYCHOTIC/INDUCED DELUSIONAL DISORDERS:
- Characterized by spread of delusions from one person to another.
- Delusional individual (primary case) typically is influential member of close relationship with suggestible person (secondary case) developing delusion.
- “Folie a deux” – Involvement of 2 people.
- “Folie a trols, folie a quatre” – Involvement of > than 2 people.
- Delusional individual (primary case) typically is influential member of close relationship with suggestible person (secondary case) developing delusion.
- Treatment: DOC – Antipsychotics.
Exam Important
- Acute psychotic disorders are frequently preceded by stressor (stressful life event) & precipitated by fever.
- Antipsychotics are DOC for shared psychotic disorder
