CATARACT SURGERY- COMPLICATIONS
COMPLICATIONS OF CATARACT SURGERY
Divided into:
- Related to Surgery
- Related to IOL
COMPLICATIONS RELATED TO SURGERY
There are so many complications of cataract surgery:
- After cataract (opacification of capsule)
- Vitreous prolapse
- Cystoid macular edema
- Endophthalmitis
- Aphakia glaucoma
- Fibrous & endothelial growth
- Retinal detachment
- Neovascular glaucoma
- Anterior uveitis (Iridocyclitis)
- Iris prolapse
- Stria keratopathy
- Corneal endothelial damage
- Most of these complications are more common with ICCE
- Except for after cataract which occurs in ECCE & is the most common complication of ECCE.
- ECCE decreases the chances of all other complications.
COMPLICATIONS RELATED TO IOL
1. Malposition of lens leads to
- Sunset syndrome (inferior subluxation of IOL)
- Sunrise syndrome (superior subluxation of IOL)
- Lost lens syndrome (Complete dislocation of IOL into the vitreous cavity)
- Windshiled wiper syndrome (results when a very small IOL is placed vertically in the sulcus)
2. Toxic lens syndrome
- Uveal inflammation.
- Excited by either the ethylene gas used for sterlising IOL (in early case) or by toxic lens material (in late case).
3. UGH Syndrome (Uveitis. Glaucoma, Hyphaemia)
- Occurring with rigid anterior chamber lens.
4. Complications like:
- cystoid macular oedema
- corneal endothelial damage
- uveitis
- secondary glaucoma
Exam Important
- Most common complication of extracapsular cataract surgery is Opacification of posterior capsule.
Causes of complicated cataract is:
- Pars planitis
- Retinitis pigmentosa
- Retinal detachment
- Infective complications in cataract operation can be decreased by Antibiotic eye drops & Intraoperative antibiotics.
- Late onset endophthalmitis after cataract surgery is most often caused by Propionobacterium Aenes.
- Malignant glaucoma is seen in after surgery for cataract or glaucoma.
