LUNG SEQUESTRATION

  • Normally developed bronchioles and alveoli
  • Supplied by systemic rather than pulmonary arteries(thoracic or abdominal aorta)
  • Commonly in lower lobes

Intralobar sequestration

  • Drain through pulmonary veins
  • In communication with airway
  • Prone to infection
  • Treatment – lobectomy

Extralobar sequestration

  • Drain to azygos veins
  • No communication with lungs
  • Asymptomatic
  • Treatment – excision
Exam Question
 
  • Sequestration of lung is seen in Posterior part of left lung
  • Abdominal aorta supply to the segment involved in intralobar sequestration of the lung
  • Bronchogenic sequestration is seen in Left lower lobe
  • Chronic left lower lobe pneumonitis  On contrast bronchography, the area involved with the pneumonitis does not fill whereas the area around it does fill. The most likely diagnosis Pulmonary sequestration
  • Intralobar sequestration of lung takes its blood supply from  Descending abdominal aorta
  • Diagnosis of lung sequestration by CT scan
  • Investigation of choice for diagnosing lung sequestration is Angiography
