REIDEL’S THYROIDITIS

• Also called as Ligneus thyroiditis or invasive fibrous thyroiditis
• Reidel thyroiditis is a very rare painless cause of a goitre with a collagen disorder (woody).
• Associated with-

1. Retro- orbital fibrosis
2. Peri- orbital fibrosis
3. Retro- peritoneal fibrosis
4. Sclerosing cholangitis

• Thyroid tissue replaced by fibrous tissue.

CLINICAL FEATURES-

• Seen in old age.
• There is intrathyroid fibrosis
• Involvement of trachea, oesophagus, IJV, carotid artery results in dysphagia and dyspnoea.
• Riedel’s thyroiditis is one manifestation of a systemic disease that can affect many organ systems called IgG4-related disease. 
• Hypothyoidism and hypoparathyroidism
• Hoarseness of voice
• Goitre is very hard , fixed, irregular and non tender.

INVESTIGATIONS-

• Wedge shaped excision biopsy.

TREATMENT-

• Isthumectomy to relieve airway compression
• Lifelong thyroixine, high- dose steroid, taxoxifen.
