Physiology of Mid dilated pupil & Iris

Tense person sitting in the dark

Mid dilated pupil

  1. Increase in the physiological pupil block

  2. Dilatation of pupil Renders peripheral iris more flaccid

More aqueous in the posterior chamber

  • Increase in IOP
  • ↑IOP causes iris bombe

Apposition of iris to lens, preventing aqueous from flowing from the posterior to the anterior chamber

  1. Angle obstruction by peripheral iris & rise in IOP
  • The pressure in the posterior chamber rises, resulting in anterior bowing of the peripheral iris and obstruction of the trabecular meshwork.
